St. Joseph Hospice - New Orleans, LAis seeking PRN On-Call Licensed Practical Nurses (LPN) to join our patient care team.

Essential duties include:

• Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
• Great time management skills

Qualifications:

• Current Licensed Practical Nurse (LPN) in the State of Louisiana
• Minimum of 3 years as a Licensed Practical Nurse (LPN) of varied work experience, two (2) years in acute care setting and one (1) year in home care or hospice preferred.
• Have dependable transportation, driver’s license and car insurance
• Previous Hospice or Home Health experience required
